Home care agencies, assisted living residences, adult day programs and long-term care facilities are inspected by state health departments, and the findings are issued as written statements of deficiencies. The provider's response, including a plan of correction, becomes part of the regulatory record and can affect licensure, ratings, referrals and payor relationships.

We review the cited findings against the applicable regulations and the facts, identify findings that can be challenged, and prepare a response that corrects real problems without admitting more than the record supports. Where a finding rests on a misreading of the regulation or incomplete documentation, we contest it.

When a survey leads to enforcement, such as fines, directed corrective action, restrictions on admissions or proposed license action, we represent the provider in negotiations and administrative proceedings and pursue the available appeals. We also help clients prepare for surveys in advance through mock surveys, policy review and staff preparation.

Because a survey outcome often overlaps with payor audits and resident or family complaints, we coordinate the response across all of them so the provider's positions stay consistent.
